Enrollment at Eyas Landing

Here are the key people you will meet during our enrollment process.

1. Office Manager

The Office Manager is responsible for initial contact, arranging clinic tours, confirming enrollment and start date, and will be your in-office advocate for your needs throughout enrollment at Eyas Landing.

2. Clinic Director

The Clinic Director may provide a personalized tour of our program and facility based on family request.

 3. Special Education Director

The Special Education Director provides a personalized tour of the Merlin program and facility for all in the Merlin enrollment process.

4. Administrative Assistant

Your assigned Administrative Assistant will provide a personalized verification of benefits, estimate of coverage with payment options, and schedule your child for an evaluation and ongoing services. This Administrative Assistant will manage all of the intake, scheduling, and billing for your child throughout enrollment.

5. Therapist/Therapy Team Lead

Your assigned Therapist or Therapy Team Lead will complete the evaluation with your child and will be the team lead for all treatment, recommendations, and clinical communication throughout enrollment.

Guide to our Enrollment Process

Step 1: Complete the registration form and speak to our Office Manager.

If you were previously a Blue Bird Day client, complete the authorization to exchange Protected Health Information form to allow the Blue Bird team to communicate their recommendation to the Eyas Landing team. The BBD Therapy Case Manager will then complete the Eyas Landing registration form on your behalf.

Step 2: Access your personal client portal to upload a copy of your insurance card in order to receive your personal benefit verification. (See our Guide to Healthcare Coverage for more information on insurance.)

Step 3: Schedule and complete a tour with our Clinic Director.

Step 4: Receive a verification of benefits and estimated cost breakdown of patient responsibility for Eyas clients for the program and payment options from your Administrative Assistant.

Summer Camps: Save your spot with deposit and registration and receive a tentative start date from our Office Manager.

Step 5: Complete the Enrollment Packet and the Credit Card Authorization with your Administrative Assistant.

Step 7: Upload all past evaluations and medical documents, ie. asthma/allergy action plans, medication consent to your CentralReach portal with your Administrative Assistant.

Step 8: Schedule and complete Evaluation scheduled by your Administrative Assistant.

Step 9: Receive individualized schedule and confirmed start date from your Administrative Assistant and Office Manager.

Step 10: Start Eyas Landing. Now you can watch your child grow!

Blue Bird Day to Eyas Landing Enrollment Process

Family therapy

1. Blue Bird Day Therapy Recommendation

Your child’s Blue Bird Day Therapy Case Manager is available to discuss and recommend therapeutic options for your family’s transition out of Blue Bird Day programming and into Eyas Landing programming as you prepare for your child’s Blue Bird Day graduation. Please Note: a recommendation is not a guarantee of enrollment. All recommendations are accommodated on a first-come, first-served basis based on availability.

2. Consent and Program Enrollment Request

The Blue Bird Day Therapy Case Manager will assist you in filling out our consent to share information and program request form for your insurance information, treatment data, and therapeutic requests to be sent to the Eyas Landing team to initiate enrollment steps.

 3. Connect with Eyas Landing Office Manager

The Eyas Landing Office Manager and intake team will provide you with enrollment paperwork, as well as a verification of insurance benefits document to review any estimated service costs for your selected programming. The Office Manager will also schedule a personalized tour with our Clinic Director or Merlin Program Lead. Please Note: a tour is required for Merlin program enrollment, but optional for private programming enrollment.

4. Eyas Landing Scheduling Administrative Assistant Assignment

Your assigned Scheduling Administrative Assistant will schedule your child for an evaluation (if needed) and ongoing services or provide you with waitlist data in the event your program choice is not immediately available. Your Scheduling Administrative Assistant will manage all of your scheduling needs throughout enrollment.

5. Eyas Landing Billing Administrative Assistant Assignment

Your assigned Billing Administrative Assistant will manage your service billing, insurance authorizations, claims, and invoicing throughout enrollment. Your Billing Administrative Assistant will be your point of contact for questions related to your invoices, cost breakdowns, or ongoing payment for service items.

6. Eyas Landing Therapist/Therapy Case Manager Assignment

Your assigned Eyas Landing Therapist or Therapy Case Manager will assist you in navigating the evaluation and ongoing treatment process with your child and will be your primary point of contact for all treatment, recommendations, and clinical communications throughout enrollment.
